Clement - Clem - McDonald was resident in a children's home in Scotland in 1965. One night he and a number of his friends were taken to a remote area where they were handed over to Captain Jack Harkness. He had been placed in charge of a top secret mission involving an alien race known only as the 456, from the frequency they used to make contact. The children were to be handed over to the aliens in return for medical knowledge - but Clem managed to escape.
The event traumatised him and he grew up to have mental health issues. After living in Leeds under an assumed name - Timothy White, from the chain of chemist shops - he ended up in a psychiatric hospital south of London.
He had verbal and physical tics, coupled with a heightened sense of smell, and some psychic abilities. He was psychically linked to the 456, sensing when they returned in 2009. As the children of Earth began chanting under the alien influence, Clem was affected in the same way - which attracted the attention of Torchwood. Clem recognised Jack from that night in 1965. Gwen deduced that the 456 had stopped hunting for him then due to his growing too old for their purposes. They were addicted to certain human hormones only present in younger children.
On being confronted with Jack, Clem shot him dead - only to see him recover.
The 456 attacked Clem with a hypersonic signal at the same time that they released a toxic gas into Thames House where one of their number had arrived as an ambassador. Clem died as they then severed the psychic link to him.
